Change personal information – Office of Student Records

Current students can update their personal information by submitting a request through SIS Self-Service. Information that can be changed includes citizenship status, date of birth, gender, race and/or ethnicity, social security number, and legal name. Students can also update their chosen name through their myJH account.

• Legal Name Change: In SIS Self-Service, The Identity Management Form is located under Personal Info > Summary > Required Information > Identity Management Form
• Chosen Name: In myJH, your chosen name is located under “myProfile” and you can modify the values in Chosen First Name, Chosen Middle Name and Chosen Last Name
