Forms and resources – Office of Student Records
This page includes some of the most common forms submitted by students. Additional forms and resources can be found here. Please refer to the Academic Calendar for dates regarding enrollment and submission of forms.
Students can add/drop classes directly through SIS Self-Service. If the add/drop request is for a course at a division other than your home school, please use the Interdivisional registration (IDR) form. Degree-seeking students at the School of Nursing may register for courses at another division at Johns Hopkins University during the fall and spring semesters. If you would like to take an interdivisional course in the summer, you must apply at that division as non-degree seeking student. Details regarding interdivisional registration can be found here.
A student may leave the Johns Hopkins School of Nursing (JHSON) by either taking a leave of absence (LOA) or by withdrawing from the school.
- A Leave of Absence (LOA) is an approved, temporary interruption of a degree program, typically lasting up to 1 year. Students may request an LOA for various reasons, including medical, military service, personal hardship, or other significant circumstances. For medical LOAs, students are required to provide documentation from a licensed healthcare provider both when initiating and upon returning from the leave. During the LOA, students are not enrolled in courses and are not considered active students, which may affect access to university services and facilities.
- A withdrawal is a complete departure from the School of Nursing and its degree program. A withdrawal leads to the closure of the student record, and can only be reversed through reapplication to the program.
Students are encouraged to consult with their academic advisor or the Office of Student Affairs to discuss the implications of each option and to receive guidance tailored to their individual circumstances.
Students that are unable to complete their coursework before the end of term may request an Incomplete grade from their instructor. Please review the details provided in the link below before submitting an Incomplete Grade Contract through SIS Self-Service.
If you are a DNP AP student who wishes to switch tracks or if you are a student enrolled in a Dual Degree program and wish to withdraw from one program, please review the appropriate form and meet with your program advisor.
