CAPABLE Transplant
Overview
CAPABLE Transplant is a home-based program made for adults waiting for a kidney transplant. Over four months, participants get help from a registered nurse, an occupational therapist, and a handy worker. Together, they work on managing symptoms, making the home safer and more organized, and getting ready for transplant.
This program is designed for people on the transplant list who may have more health problems, feel tired often, or face challenges from dialysis.

Main Goals of the Study
Goal 1: Adapt the CAPABLE program to meet the needs of people on the kidney transplant waitlist. This will include interviews with patients and healthcare providers, feedback from community members, and creating and testing new ideas.
Goal 2: Try the adapted CAPABLE program with a small group of patients who are on the waitlist but not yet active. We will see if the program is easy to use, if people are happy with it, and how we can make it better.
Why It Matters
- People with many symptoms have a 67% higher risk of dying while on the waitlist.
- Being “inactive” on the waitlist means longer waits and worse health outcomes.
- Challenges like limited social support, navigating the healthcare system and incomplete testing keep many people from getting the care they need.
The goal of CAPABLE Transplant is to bring care directly to patients at home to help them stay healthier and move toward a successful transplant.
What’s Included:
- 6 Home Visits over 4 months
- Minor Home Modifications
- Free Internet, iPad, and tech training if that is a goal chosen
- Goal Based on what matter most to the participant
- Receive up to $100 for full participation
You could be eligible if:
- 18 years or older AND
- Live within 40 minutes of Baltimore City AND
- Your waitlist status:
- Either active on the waitlist & low- income OR
- inactive on waitlist OR
- active and have been inactive in past 18 months

- Improve the quality and security of our services
3. SMS Messaging and Compliance
If you opt in to receive text messages:
- Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes.
- Text messaging originator opt-in data and consent will not be shared with third parties except as needed to provide messaging services.
- We may share information with our SMS delivery providers (e.g., Telzio) only as necessary for technical enablement, under strict confidentiality obligations.
4. Opt-Out Instructions
You can stop receiving texts at any time by replying to any message with STOP.
Once we receive your request, we will stop sending SMS messages to your number.
Standard carrier message and data rates may apply.
5. Data Sharing and Third Parties
We do not sell, rent, or share your personal information with third parties for their own marketing purposes.
We may share information with trusted third-party providers who support business operations, including:
- SMS/text message delivery providers
- Website hosting and analytics services
- Customer support platforms
All such providers are contractually required to safeguard your information.
6. Data Security
We implement industry-standard administrative, technical, and physical safeguards to protect your personal data from unauthorized access, loss, misuse, or disclosure. Only authorized members of the research team have access to identifiable messaging data.
7. Your Rights
You may request access to, correction of, or deletion of the personal information we maintain about you, as allowed by law or research regulations.
You may also withdraw consent for messaging at any time by replying STOP or contacting us directly.
8. Changes to This Policy
