Health Passports

These Health Passports were created by the COMPASS Center to assist individuals in organizing their medical information. The nurses at the Wald Center provide these passports to educate patients on health promotion, preventative screening, and a place to record medical information such as blood pressure reading, medications, and future appointments. Nurses may select the appropriate health passport to send to patients so they are able to electronically download the Health Passport to their devices or print the Health Passports for patients.
