Changhwan Kim is a DNP/PhD student with a clinical background in intensive care and progressive care units in South Korea. His research focuses on hospital-to-home transitions among acute and critical illness survivors and their care partners, examining how transition quality and system-level ambiguities shape self-management and caregiving capacity. His goal is to become a clinically grounded nursing translational scientist, advancing the development, implementation, and translation of patient-centered interventions to improve recovery outcomes and reduce post-discharge vulnerability among high-risk populations.
