Neighborhood Care Pathways is a research study focusing on a new community program to help us understand how we can build up the health and resilience of older adults.
We offer:
- Screening for unmet health needs and personal wellness planning
- Health education and check-ins with a care navigator
- Support finding and connecting with resources
- Over-the-counter health products and in-home services based on your needs
Whether you’re healthy, or need assistance, we have something to offer.

- Adults age 50 or over
- Living at home (not in nursing home or assisted living)
- Residing in and around the neighborhoods of Greenmount West, Johnston Square, and Oliver in Baltimore
