Neighborhood Care Pathways is a research study focusing on a new community program to help us understand how we can build up the health and resilience of older adults.

We offer:

  • Screening for unmet health needs and personal wellness planning
  • Health education and check-ins with a care navigator
  • Support finding and connecting with resources
  • Over-the-counter health products and in-home services based on your needs
