Note: Questions about the form should be directed to University Health Services at 410-955-3250. Completed forms must be received by the deadline so that University Health Services can provide you with health clearance and avoid any registration "holds." Please send completed forms to the Office of Admissions.
Johns Hopkins University's policy states all students must maintain adequate health insurance coverage. School of Nursing students may purchase insurance through the Student Health Program (SHP) or provide their own comparable health insurance coverage by completing a waiver form.

Johns Hopkins Medicine—the Johns Hopkins Hospital and Health System and the Johns Hopkins University School of Medicine—have instituted a mandatory influenza vaccination policy requiring all healthcare personnel and employees to receive a flu vaccination if they:
As a member of the Johns Hopkins healthcare family, the School of Nursing follows the same policy. All students are required to be vaccinated annually. This requirement should be completed during the fall semester. Free vaccinations will be available to all nursing students at The Johns Hopkins Hospital from early October through December.
All students must complete a criminal background check through PreCheck prior to orientation and urine drug screen within six weeks of orientation. Once complete, your full history report will automatically be sent to the Office of Admissions and Student Services. You can view your report for 90 days after completion by going to http://mystudentcheck.com.

It is highly recommended to print a copy for your records. Students are obligated to report any changes to the information appearsing in their criminal background check after matriculation to the Associate Dean for Enrollment Management and Student Affairs. Changes may need to be reviewed by the appropriate Academic Progressions Committee to assess the need for further action.

These online modules are required of all students and must be completed prior to orientation. Your JHED ID will be required for login. Please keep a copy of the completion certificate for your own records and submit another copy to the Office of Admissions to jhuson@jhu.edu, or fax to 410-614-7086 by orientation.
If you are anticipating student financial aid or loans to cover your living expenses, it is important to keep the green Higher One envelope. It contains your card--the key to unlocking your refund preference. Cards will be sent to the mailing address you registered with the School of Nursing.

Students have access to over 100 computers throughout our buildings. Many also find it convenient to own a laptop for use at home or while in the building. If you already own a computer you wish to use for school work, ideally it should be no older than four years old with sufficient processing power and memory to run current versions of Microsoft Office and Symantec Antivirus, and possibly more demanding applications such as statistical software and Second Life.
Use of personal laptops within the school is encouraged. The school supports a high-speed wireless network with coverage throughout all of the school buildings. The SON Help Desk, part of an array of IT services offered by the ITS department, offers on-site technical assistance for students, including support of student laptops. Computer Recommendations
